Share a Concern With the Council

The MINDAKS Veterans Mental Health Council exists to ensure Veterans’ experiences and concerns are heard.

If you have experienced a problem with VA mental health care, encountered a barrier to receiving services, or believe there is an issue affecting Veterans in our region, you may use this form to share your concern with the Council.

Your submission helps us identify recurring concerns, recognize gaps in services, and advocate for meaningful improvements in partnership with the Fargo VA and community stakeholders.

The MINDAKS Veterans Mental Health Council is an independent Veteran-led advisory council. We are not a clinical care team, investigative body, legal service, or replacement for the VA Patient Advocate.

If you are seeking resolution of an individual complaint directly through VA, you may also contact your VA health care team or VA Patient Advocate.

What Happens After You Submit?

A member of the MINDAKS Veterans Mental Health Council will review your submission.

Depending on the nature of the concern, the Council may:

  • Contact you for additional information, if you have requested contact.

  • Raise the issue with appropriate VA leadership or Council partners.

  • Identify the concern as part of a broader pattern affecting Veterans.

  • Recommend that you contact the appropriate VA Patient Advocate or another official resource when individual case resolution is needed.

  • Use de-identified information to help guide Council priorities and recommendations.

Submitting a concern does not guarantee that MINDAKS can resolve an individual case. Our role is to listen, identify concerns, advocate for Veterans, and help improve the systems that serve them.
